Delayed ejaculation (DE) is when a man becomes aroused and achieves a firm erection, yet finds it difficult to ejaculate in a reasonable time — or sometimes not at all. This condition can occur during intercourse or masturbation. While occasional delay may be normal, persistent inability to ejaculate can lead to frustration, stress, and impact intimacy and relationships.
DE can affect men intermittently or as a lifelong condition. For some, it always has been a challenge; for others, the problem develops later after previously normal sexual function.
Sometimes, DE occurs only under certain conditions — with some partners, or only during intercourse, but not during masturbation.
DE often arises from a mix of physical and psychological factors. Understanding the root cause is essential to selecting effective treatment.
Physical contributors may include:
Psychological or psychosocial factors may include:
Because multiple factors often overlap, a careful and holistic evaluation is key.
Adjustments to medication or lifestyle (e.g., reducing alcohol, addressing recreational drug use)
Psychological or sex-therapy (alone or with your partner) to address emotional, relational, or performance-related factors
Hormonal therapy if testosterone or other hormonal deficiency is identified
Supportive treatments such as shockwave therapy, PRP (P-Shot®), or other therapies when appropriate
In cases where a structural issue or nerve damage is suspected, we may recommend further urological evaluation.
It’s natural for ejaculation timing to vary occasionally. But if you consistently find ejaculation difficult, delayed, or absent — and this causes distress, reduces sexual satisfaction, or affects your relationship — it’s important to seek professional consultation.
